Functional Resonanse Analysis Method (FRAM) - when classic thinking is a dead end
In the classical safety view, humans are a hazard and error prone, wherefore standard accident investigations typically identify "human error" or “crew negligence” as the root cause that needs to be managed. This is one truth only, which historically has left us with a limited understanding about; how “work is carried out” and measures to prevent re-occurrences.
The FUNCTIONAL RESPONANCE ANALYSIS METHOD (FRAM) approach helps us to develop a model of the activity (process or performance), a representation and an understanding of the situation as a whole – favour asking questions, before searching answers – causes.
